Overview of .zip and .sit Compressed Files

This site contains some files for download in .zip (PC) and .sit (MAC).  A .zip/.sit file is a compressed file "shell" that contains a larger file inside.  Once downloaded, compressed files can be opened by double-clicking the file icon on your computer.  Note:  To open .zip/.sit files you must have a tool to open compressed files such as WinZip or StuffIt Expander.  You may download a free evaluation copy of WinZip at http://www.winzip.com (for PC) and StuffIt Expander (Macintosh or PC)  at http://www.stuffit.com.

Overview of Adobe Acrobat PDF Files

Make sure you have version 5.0 of Adobe Acrobat Reader to download and view PDF files on this web site. You can download Acrobat Reader FREE on the Adobe web site.

Instructions for Downloading and Filling Out Forms

  1. To view a document in PDF format, click on the document link. 
  2. To download and save the document to disk, complete the following:
    a) PC format - right-click the links and select "save target as," or
      b) Mac format - click-hold the links and select "download link to disk."
  3. To fill out the required information in the PDF form, you can do one of the following:
    a) Print the forms from either the Internet browser or your downloads. Then, fill them out by hand or use a typewriter.
      b) You can also fill out the forms on your computer. Click inside the document's form boxes, while in the browser or your download, and type in the required information on your computer. Then print out your completed form BEFORE closing it.

    IMPORTANT NOTE: Since Adobe Acrobat Reader does not allow you to save the information you've filled out in the document, you will be unable to edit your completed form once you've closed it. Therefore, before you start filling out a form, please make sure that you have all the correct and final information available.

    Alternatively, you may wish to print two forms, fill one of them out by hand and ensure its accuracy, then fill out the final form on your computer. When finished, print your completed form from the computer.

This site uses Adobe's Portable Document Format (PDF). PDF files maintain the formatting and appearance of print documents, and are easily downloaded and readable on a wide range of computer platforms.
